Registered Nurse – UAS Assessor - Hybrid

Location: Multiple locations across New York State
Pay: Starting at $50/hr (based on experience)

Are you a compassionate Registered Nurse looking for a flexible hybrid role where you can impact lives? Join the New York Independent Assessor Program and conduct UAS assessments to determine eligibility for community-based long-term care services.

3 Days In-Home Assessments | 2 Days Remote Work – A perfect balance of fieldwork and work-from-home flexibility!

What You’ll Do:

✅ Conduct initial health assessments to evaluate medical needs, daily functions, and care preferences.
✅ Visit clients 3 days per week within your local ZIP code for in-home assessments.
✅ Work remotely 2 days per week, conducting telehealth assessments and completing documentation.
✅ Accurately enter data into the UAS-NY online system.
✅ Collaborate with care teams for precise evaluations.

What You Need:

✔ Active NY RN License
✔ Minimum 2 years of home health experience (CHHA, LHCSA, MLTC, or similar)
✔ Strong proficiency with Zoom, Microsoft Word, Outlook, and web-based applications
✔ Excellent organizational & communication skills
✔ Ability to travel locally for in-home visits
✔ Reliable high-speed internet (Minimum: 20 Mbps download, 5 Mbps upload)
