Registered Nurse - New York Independent Assessor Programme
Pay: from $50 per hour, based on experience

We’re seeking passionate and experienced Registered Nurses to join the New York Independent Assessor Program, conducting UAS assessments to determine eligibility for community-based long-term services. This HYBRID position requires 3 days per week in the field conducting in-home assessments and 2 days working remotely from your home office, offering flexibility and variety in your workweek.

Multiple locations available within New York State.

What You’ll Do:

  • Conduct initial health assessments to evaluate overall health, functional needs, and care preferences.
  • Perform in-home assessments within New York State, local to your ZIP code, 3 days per week.
  • Work remotely for 2 days per week, completing telehealth assessments and documentation.
  • Input and code assessment data into the UAS-NY online system.
  • Collaborate with care teams to ensure accurate evaluations.

Requirements:

  • Active RN license in New York.
  • Minimum of 2 years of experience in home settings (e.g., CHHA, LHCSA, MLTC).
  • Proficiency with Zoom, Microsoft Word, Outlook, and web-based applications.
  • Strong organizational and communication skills.
  • Ability to travel within your assigned region to conduct in-home assessments.
  • Reliable high-speed internet (minimum 20 Mbps download, 5 Mbps upload).

Schedule Details:

  • 3 days per week in the field visiting consumers' homes.
  • 2 days per week working remotely from your home conducting telehealth assessments.
